Healthcare Provider Framework for Early Detection and Diagnosis of Cognitive Impairment

This framework provides a clear, step-by-step approach to identifying and diagnosing cognitive impairment. It outlines roles and responsibilities across primary care and specialty care, offers co-management recommendations, and supports timely movement from concern to diagnosis. This framework is for both the primary care team (i.e., primary care physicians, nurse practitioners, social workers, physician assistants, and nurses) and specialists.

Informed by Expertise and Lived Experience

These resources were made with direct input from people with lived experience and professional expertise. UsAgainstAlzheimer's convened two advisory groups that provided ongoing feedback through group discussions, one-on-one conversations, and document review.

A human-centered design firm led design efforts and regularly obtained feedback at every stage from both advisory groups. This process enhanced and refined tools to further ensure that the tools are practical, respectful, and responsive to real-world needs.  Learn more about the committee members below.

Expert Discussion
  • Lived experience advisory group: People living with mild cognitive impairment due to Alzheimer’s disease, people living with increased risk of Alzheimer’s disease (APOE e4 carriers), current and former caregivers to family members with Alzheimer’s disease, licensed clinical social workers, etc.
  • Expert advisory group: dementia experts, general neurologists, primary care providers, specialists, researchers, licensed clinical social workers, and nurses.

UsAgainstAlzheimer’s is a nonprofit patient advocacy organization which aims to catalyze change in the health ecosystem to improve dementia prevention, early detection, and access to treatments through advocacy, policy, convening, and high-leveraged programs, with a particular focus on health equity.
